Physicians and nurses on Unit 5E (Family Medicine) now use a standard provincial form to ensure changes and additions to a patient’s medications during
a hospital stay are communicated to the patient, family, and other health care providers upon discharge.
The form, to be rolled out region-wide, reduces confusion and improves safety for patients.
