A small number of referral and follow-up letters to patients were delayed in their distribution recently. These letters include referrals to, and follow-up letters from, specialists and the Saskatchewan Cancer Agency.

We have become aware of a process issue that occurred in dictation and transcription whereby a small number of referral and follow-up letters were delayed in their distribution. These letters include referrals to, and follow-up letters from, specialists and the Saskatchewan Cancer Agency.

We took immediate measures to correct this issue and we are directly contacting all patients who were affected. We currently know of 250 patients throughout the province who may have been impacted. 3sHealth is doing additional auditing to determine if there are any further cases.

We are also contacting all physicians directly affected by this in order to ensure proper investigation and assessment into any potential for harm to the patients affected.

I want to make clear that there is no specific action required as a result of this communication. If one of your patients was affected, we will contact you directly with the information necessary for you to follow up.

We wanted you to be aware of this issue because we will be following up by doing a more thorough root cause analysis of the problem to identify areas for improvement. So if you have experienced any problems with dictation and transcription processes, please feel free to follow up at the number provided below so these concerns can be logged and addressed as part of this work.
