Make sure you're completing MVLSTS correctly

IS THE MVLST YOU COMPLETED LEGAL?
HIMS removes incomplete MVLSTS from a patient's charts and sends them to the Advance Care Planning Program (ACP) office for review. Numerous incomplete forms are removed from patient charts each week and returned to the ACP office because they are not completed according to the ACP work standard. MVLST forms are not considered legal if some key components are overlooked.

6 KEY POINTS TO ENSURE THE MVLST IS COMPLETE AND LEGAL:

  • The chosen Goal of Care needs to be initialed by the patient
  • Does the patient have any specific directions to be noted on the form?
  • Is the person who assisted the patient complete the form identified in the MVLST?
  • Has the patient or substitute decision maker signed the form?
  • Has the Health Care Professional signed the form?
  • Has the form been dated?

Did you know the Advance Care Planning Record is equally as important as the MLVST form?
Our patients share their healthcare wishes with their healthcare team. It is standard practice in our day to day work with our patients to record their wishes correctly to ensure we follow their goals of care should they become unable to speak for themselves. Upon discharge a copy of the MVLST should be provided the patient or legal health care substitute decision maker for their records.
