The Region's Long Term Care program offers long term care, sub-acute care and day support programs in rural areas of the Region including the following:

• Balcarres, Saskatchewan
  • The Balcarres Integrated Care Centre offers long term care, respite and adult day care services.
  
  
• Broadview, Saskatchewan
  • Broadview and District Centennial Lodge is a 36-bed long term care facility. The facility offers an Adult Day Support Program.
  
  
• Cupar, Saskatchewan
  • The Cupar and District Nursing Home is a 48-bed long-term care facility.
  • Adult Day Support Program is offered at the nursing home.
  
  
• Fort Qu'Appelle, Saskatchewan
  • Echo Lodge has 50 permanent long term care residents, and provides respite and adult day care.
  
  
• Grenfell, Saskatchewan
  • Grenfell and District Pioneer Home is a 34-bed long term care facility. The facility offers an Adult Day Support Program.
  
  
• Imperial, Saskatchewan
  • Long Lake Valley Integrated Facility; this facility offers permanent placement, convalescent/respite/palliatives and observation.
  • As needed, the facility also offers adult day support services.
  
  
• Indian Head
  • The Golden Prairie Home in Indian Head is a 38-bed long term care facility. The facility offers an Adult Day Support Program.

  • St. Joseph's Integrated Care Centre offers long term care, respite and adult day care services.
  
  
• Lumsden, Saskatchewan
  • The Lumsden and District Heritage Home is a 30-bed long term care facility located in the Township of Lumsden.
  • This facility offers an adult day support program.
  • Assisted Living Services at the Lumsden Manor (Senior Housing), is also available in this community. Services include the availability of a support worker and coordinated meal service.

  • The Southeast Integrated Care Centre is a 58-bed long term care facility. The facility offers an Adult Day Support Program.

  • Silver Heights Special Care Home has 29 permanent residents and provides respite care and adult day care.
