Long Term Care Services: Convalescent Care Program The Convalescent Care Program assists individuals with recuperation and recovery after surgery, or serious illness. The Region has designated beds in long term care facilities for this purpose. The average length of stay is between two to six weeks. Discharge from care may occur sooner depending on an individual's health status. Only those individuals returning to the community are admitted into the program. Based on program criteria, SWADD determines eligibility and coordinates admissions into the convalescent care program. The interdisciplinary team consists of SWADD, the individual and their caregiver, the family physician, physiotherapist, nurses, and other facility staff. The team provides:
Convalescent care costs are based on a fee schedule, similar to the fee schedule for home care. Additional costs for transportation, clothing, personal grooming items, medications and medical supplies, incontinence supplies, supplies for oxygen therapy, mobility equipment, dental and prosthetic devices, and sensory devices (glasses, hearing aids, etc.) are not covered by the accommodation fee.
