Diabetes Services: Home Care

Home Care provides health and support services for people of all ages who wish to remain independent in their own homes. Services are intended to support and supplement families and other community resources, which may permit earlier discharge from hospital, support ongoing long-term care needs in the community or prevent institutional placement. The range of services available depends on the assessed need of the individual but may range from service once weekly to daily. Service is available throughout a 24-hour period, seven days a week.

People with diabetes may require and receive a variety of home care services from the RQHR. Services may include:

• Medication management, insulin administration, preloading of insulin syringes, blood sugar and blood pressure monitoring along with diabetes education.
• Wound care and dressing changes if required.
• Assistance with personal care/home management
• Meals on Wheels or assistance with meal preparation
