Transition Unit

Transition Unit care services are available to individuals in hospital who have care needs that are consistent with requiring placement in a special care home, but have some potential for improvement to return to the community. Admission to the Transition Unit is based on program criteria and access is coordinated by the intake coordinator in your area. Admission is determined prior to discharge from hospital.

There are 12 Transition Unit beds located at Extendicare Parkside. Care is provided 24 hours a day, seven days a week. The average length of stay in transition care is between 30 to 45 days, with amaximum stay of three months.

A team of staff in the facility provide:

  • assessment services;
  • nursing care;
  • physical therapy;
  • client and caregiver teaching and support;
  • pastoral services (optional);
  • recreation and socialization opportunities;
  • nutrition, laundry and housekeeping services;
  • and medical services in collaboration with the family physician, including confirmation of orders, health assessments, referral to specialists, and diagnostic services.

The Transition Unit fees are the same as all other residents' fees in Extendicare Parkside. Other costs not covered by the accommodation fee include: transportation, clothing, personal grooming items, medications and medical supplies, incontinence supplies, supplies for oxygen therapy, mobility equipment, dental and prosthetic devices, costs for marking personal belongings.

RQHR SWADD will assist in arranging transportation from hospital to home, or to the transition care unit. Costs for transportation are the responsibility of the individual.
