Meadow Primary Health Care Centre is located in Regina’s inner city, directly across from the Pasqua Hospital at 4006 Dewdney Avenue. Led by the Regina Qu’Appelle Health Region (RQHR) and four family physicians, the centre’s team of health care providers will provide primary health care services to Regina’s inner city neighbourhoods, in addition to providing visiting services to the rural locations of Regina Beach, Southey and Cupar. Among its offerings, the centre will focus on meeting clients’ unique health care needs — including chronic disease management — and provide extended hours for immediate care.

RQHR worked closely with clients from the community to design the centre. Their voice was very important as it shaped the design of the centre and helped determine the services the centre offers.

The Meadow got its name from RQHR clients. During a 3P Kaizen event in March 2012, the clients participating in the event decided on the name Meadow because much like a meadow of many different types of flowers, the Health Region needs to be responsive to all clients and provide services to a diverse population with a wide range of health care needs. One of the community participants at the Kaizen event, spoke up and said the people coming to this centre will be like flowers a in a meadow – many different colours and types, all unique and beautiful.

What is Primary Health Care?
Primary Health Care is a philosophy and approach in which a team of health professionals provide everyday health services focused on working with patients, clients, families and communities.

What is an innovation site?
An innovation site is a facility that focuses on meeting clients’ unique health care needs – including disease management. An innovation site explores new ways to deliver team-based health care to improve access and patient experience. In Regina the focus is on improving the patient experience for inner city residents.

The Meadow will improve access and connectivity in primary health care, by providing new ways of delivering health care services that meet the specific needs of the community. The Meadow will also offer enhanced access to after-hours care and link to other services available to clients.

The Meadow will provide the provision of everyday medical care such as: physicals, prenatal care, and disease and illness prevention. The centre will also offer immediate care services such as; treatment of episodic illness or injuries, minor injuries such as sprains and strains and stiches.

In addition, the centre will offer chronic disease management services aimed at promoting self-management of conditions such as diabetes, COPD or depression.
