General Instructions for Submission

  • Applications that require full board review (above minimal risk studies) must be received at least 10 business days prior to a scheduled meeting in order to be reviewed at the upcoming meeting; however, there are instances when applications are required earlier (e.g., due to planned office closures). The Research Ethics Board (REB) typically meets at 12:00 pm on the second Monday of every month at Wascana Rehabilitation Centre.
    • Applications must be complete (including all required signatures) in order to qualify for review at the next meeting of the Research Ethics Board. If you would like to have your application for full board review screened by a member of our office prior to your submission to the REB, it is recommended that you contact ResearchEthics@rqhealth.ca with a request for screening no later than 2 business days prior to the submission deadline.
  • Applications that do not require full board review (i.e., minimal risk studies that meet the criteria for delegated review, as outlined in the guidance notes linked in the ethics application form) may be submitted at any time and will be reviewed on an ongoing basis.

How to submit your application

All applications for ethical approval of research proposals (i.e., those requiring full board review and those qualifying for delegated review) are made by submitting ONE electronic copy of the completed application form and all attachments in PDF format. The electronic copy of your application and all related files should be sent to ResearchEthics@rqhealth.ca. Hard copies of complete applications are no longer required. Note: If your application falls within the jurisdiction of more than one Saskatchewan REB, see “Submitting to Two or More REBs”.

What to Include:

  1. Application for research ethics review (with guidance notes removed).
  2. RQHR departmental/operational approval form (currently N/A for prospective biomedical applications, as this is included within the application form)
  3. All applicable recruitment materials, consent forms, data collection tools, master lists, investigator brochures, etc.
  4. Supporting documentation (e.g., Health Canada No Objection Letter, approvals from other REBs)
  5. Required ethics tutorial certificates of completion
  6. McMaster Chart Audit Tutorial https://ethics.mcmaster.ca/chart/: Required for PI’s
  7. Tri-Council Policy Statement: Ethical Conduct for Research Involving Humans (TCPS) tutorialhttp://www.pre.ethics.gc.ca/eng/education/tutorial-didacticiel/: Required for PI’s of studies involving contact with patients (e.g. patient interviews, patient surveys, focus groups, etc.)
  8. CV for Principal Investigator (for prospective biomedical studies)
  9. Signatures
  • All signatures requested must be obtained prior to submission to the Board. Applications with missing signatures will be returned to the investigator without review. Please submit signature pages as scanned PDF pages of hand written signatures. Typed text submitted in lieu of a signature will not be accepted.
  • If your project is above minimal risk, the original signatures on the application form (Principal Investigator, Department Head, and Student Investigator, if applicable) must be submitted in hard copy to the REB. Signature pages can be mailed to: Research Ethics Board, 2180 – 23rd Ave. Room M-704, Regina, SK, S4S 0A5. Operational approval signatures may still be submitted as scanned PDF files, however.
