Potential Primary Health Services include:

primary medical care, emergency medical services, community mental health, addictions, public health (population health), health promotion, nutrition, supportive care (i.e. special care homes, respite care, adult day care), home care, end-of-life care (palliative care), laboratory and x-ray services, support for informal caregivers, therapy services (i.e. physio, occupational, speech and language) and prevention/treatment of chronic diseases.

Your PHC team could include:

family physicians, nurse practitioners, public health nurses, nutritionists/dietitians, physiotherapists, occupational therapists, social workers, dentists, chiropodists, chiropractors, elders, volunteer caregivers, home care nurses and workers, mental health nurses and workers, addictions workers, psychologists, optometrists, pharmacists, paramedics/emergency medical technicians, exercise and fitness specialists and others.
